During Hell's Gates, Heaven's Collide, SUB ZERO, and Party, equip a character from your hotbar so it has a chance of being mutated. And if you've run across mentions of a "skyfall potion": no potion by that name exists — the Gems and Stars falling from the sky belong to the Heaven's Collide and Hell's Gates events.
